Case Manager

Purpose of Position: Provide direct services to persons living with HIV, including, but not limited to eight core services: intake, assessment, care plan development, treatment adherence counseling, resource development, service coordination, monitoring, reassessment, and discharge planning.

Primary Tasks:

  • Coordinates and completes the intake, assessment, care plan development, resource development and referral, monitoring and reassessment, and discharge planning for all clients assigned.
  • Refers clients for medical care and basic needs and seeks to address psychosocial issues, housing issues, legal concerns, and other issues specific to individual client needs.
  • Provides information to clients about the transmission of HIV from person to person and offers specific information on how to reduce HIV transmission.
  • Assists in providing pre-and post-test counseling for people being tested for HIV.
  • Ensures confidentiality of clients by guarding against disclosure of HIV status without informed and written consent of clients. Follows all Federal HIPAA standards in agency work.
  • Maintains accurate, professional, and complete documentation of services provided.
  • Attends community meetings as appropriate to assist in continuity of care for all clients and represents THP in all settings in a professional manner.
  • Assures professional development by enrolling in continuing education on issues related to HIV care and prevention. Attends at least twelve hours of training per year that is approved by the AIDS Care Branch of NC.
  • Abides by all policies and procedures of THP.
  • Performs other tasks as assigned by the Executive Director, the Associate Director, the Client Care & Wellness Manager, and the Staff Care & Wellness Manager that contribute to the Agency’s functioning.

In addition to these tasks, every Triad Health Project employee is asked to participate in fundraising activities for the organization. These activities vary and are held throughout the year. Additionally, all Triad Health Project staff, interns, and volunteers are asked to commit to our organizational pursuits of equity and harm reduction, in which require organizational training, reading, and processing at various times. Outreach and fundraising activities may include evening and weekend hours.

Minimum Qualifications:

  • 4-year degree from an accredited college or university in Social Work or other related human service field
  • Associate's Degree with 2 years of work experience within the human service field preferred
  • Ability to fluently read, write, and speak in English and Spanish preferred

Possess the ability to work with the public in a professional manner, handle all clients and support networks with cultural competence and non-judgmental attitudes, develop therapeutic relationships with clients, work with teams, use professional judgment, work independently, communicate orally and in writing in a professional manner, and work with a flexible work schedule.

    • 8 hour shift

    Salary and Benefits

    $39,500-$43,500 Starting Salary ranges are based on experience in the field.
