The Intake & Outreach Coordinator plays a pivotal role in ensuring families connect with SAFEchild’s programs and essential resources addressing social determinants of health, including food security, housing, childcare, and transportation. This position manages the intake and referral process, strengthens outreach and partnerships, and raises awareness about SAFEchild’s services.
The ideal candidate has strong organizational skills, relationship-building abilities, and a commitment to trauma-informed, family-centered support. This is a full-time (40 hours per week) position reporting to the Program Director. SAFEchild is committed to the principles and practices of creating a respectful and welcoming workplace culture.
Key Responsibilities
Intake Coordination
- Serve as the primary point of contact for families seeking SAFEchild services through phone, email, and website inquiries.
- Create a welcoming, trauma-informed experience for families from their first interaction.
- Provide a clear overview of SAFEchild’s programs, guiding families through the registration and enrollment process.
- Assign families to appropriate programs while maintaining accurate rosters and documentation.
- Track and document intake and referral data to monitor families’ engagement from intake to program completion.
- Maintain intake and registration data in Salesforce, updating processes to improve efficiency.
- Analyze intake trends and generate quarterly reports on referral sources and program engagement.
- Work with Salesforce to ensure that registration forms remain up-to-date and accessible.
- Collaborate with program coordinators to ensure smooth transitions for families from intake to program participation.
Outreach Coordination
- Build and maintain strong partnerships with referral sources, including Wake County Human Services, the judicial system, WCPSS, and mental health providers.
- Represent SAFEchild at meetings, conferences, and outreach events to increase awareness of available services.
- Maintain an up-to-date community resource directory for families and staff through collaboration with external partners.
- Addressing Social Determinants of Health (SDOH)
- Conduct family needs assessments to identify barriers related to food insecurity, transportation, childcare, housing and others.
- Develop individualized resource plans to connect families with critical services and community supports.
- Cultivate and maintain relationships with local organizations, including food banks, housing authorities, and transportation providers.
- Monitor and report on SDOH trends to inform program improvements and better meet family needs.
Additional Responsibilities
- Perform additional tasks as assigned by the Program Director or Executive Director.
Qualifications
- Bachelor’s degree required in social work, public health, psychology, human services, or a related field.
- 3+ years of experience in intake coordination, community engagement, case management, or child maltreatment prevention.
- Strong verbal and written communication skills to effectively engage families, staff, and external partners.
- Proficiency in Microsoft Office 365 (Word, Excel, PowerPoint, SharePoint) and experience using Salesforce or similar client management systems.
- Strong organizational skills with the ability to manage multiple tasks, meet deadlines, and adapt to changing priorities.
- Experience in community outreach, partnership building, and public engagement strategies.
- Knowledge of social determinants of health and their impact on child and family well-being.
- Bilingual (English/Spanish) preferred but not required.
Work Experience
- Experience managing a multifaceted program with a large team of internal and external professionals.
- A minimum of five years of professional experience working with children and families where abuse and violence are identified issues.
- Previous professional experience working with the criminal justice or child welfare system and as a member of a multi-disciplinary team is preferred.
