  • Originally founded as Hospice of Wake County in 1979, Transitions LifeCare provides a comprehensive circle of expert care and support. Our philosophy is that every individual deserves the quality of life he or she wants when facing a serious illness or near life’s end. Our mission is empowering individuals, families and communities to embrace meaning throughout a chronic serious illness and at the end of life.

  • To secure for all people with intellectual and other developmental disabilities the opportunity to choose and realize their goals of where and how they learn, live, work and play.

  • To build upon the social, physical, emotional, and spiritual strengths of children, youth, and families and to affirm their worth.

  • To provide the nurturing care that will heal a child's broken spirit and help reunite families who have experienced discord and separation.

  • To assist families in need by providing a safe home-like setting for children until they are reunited with family or achieve independence.

  • To provide an affordable, safe, nurturing home away from home for seriously ill patients, their family members, and caregivers throughout NC and beyond.

  • To serve the community as a food pantry and outreach.

  • To alleviate hunger in Beaufort County

  • To serve, through cooperative community action, our neighbors in Rowan County who are in crisis, by providing essential life needs and educating and empowering them to break the cycle of crisis.

  • To provide immediate relief from financial crisis by assisting with rent, utilities, food, clothing, and prescription medications. Also, to provide case management to help individuals reach self sufficiency.
