Housing Stabilization Specialist — Triangle Family Services

Work rooted in meaningful, relationship-based connection: Most days, you’ll be in the community and conducting home visits as part of direct case management support focused on long-term housing stability and client self-sufficiency. You’ll meet clients where they are, build trusting relationships, and work alongside them to identify strengths, remove barriers, and develop skills and resources for successful independent living.

The Role: As a Housing Stabilization Specialist (HSS) on our Homeless Engagement Team, you’ll be a lifeline for people who are working toward housing stability. That means:
· Meeting clients at temporary locations or in their homes and collaborating with them to develop person-centered service and stability plans
· Cultivating collaborative relationships with landlords to increase client housing options and support positive rental experiences
· Connecting individuals and families to the community resources and supports that fit their needs
· Carrying a caseload with real, ongoing relationships — not one-and-done referrals
· Documenting your work and participating in agency and community case conferencing to ensure no one falls through the cracks
This role is a fit if you’re comfortable working alongside individuals experiencing mental illness and active substance use — and if you believe that compassion and accountability can coexist.
You Might Be Our Next Stability Case Manager If You…
· Have at least two years of experience working with people experiencing homelessness, SUD, SMI or SPMI
· Have an associate’s, bachelor’s or master’s degree in social work, human services, or a related field – or equivalent work experience
· Have a strong knowledge of community resources and navigation skills
